The Patient Care Coordinator is responsible for providing front office and patient care coordination support necessary for effective clinic operations and business functions within the medical facility. This role supports patient check‑in and check‑out, appointment scheduling, phone coverage, insurance verification and authorizations, referral coordination, and patient payment collection.

Serving as a key liaison between patients, families, clinicians, and administrative teams, the Patient Care Coordinator ensures accurate patient information, well‑managed clinician schedules, and timely communication to support a positive and efficient patient experience.
Minimum Education and Experience
Two years of college and three to five years related experience; competent computer skills; or an equivalent combination of relevant education and experience.

  • Bilingual-Fluent in Spanish and English.

  • Strong patient relations skills
  • Strong personal computer literacy, including working knowledge of Microsoft Office (Excel and Word)
  • Ability to work accurately with data, including integrity checks, summation, and effective communication to internal and external constituents
  • Strong organizational skills; ability to manage multiple activities simultaneously and prioritize effectively
  • Self-starter with the ability to work independently
  • Demonstrates a high degree of accuracy and readability in all work and team projects
  • Protects patient health information (PHI) in accordance with HIPAA standards
Other Qualifications
To the extent this position requires the holder to research, work on, or have access to critical infrastructure as defined in Section 117.001(2) of the Texas Business and Commerce Code, the ability to maintain the security or integrity of the critical infrastructure is a minimum qualification to be hired and to continue to be employed in the position.
Essential Duties and Responsibilities
Essential Duties and Responsibilities
  • Provide consistent coverage at the patient check‑in and check‑out window.
  • Answer incoming phone calls promptly and respond to patient and family inquiries with professionalism and courtesy.
  • Manage all patient check‑in and check‑out processes to ensure accurate and efficient patient flow.
  • Collect, post, and reconcile patient co‑pays and other payments at the time of service.
  • Schedule patient appointments in the medical billing system and maintain accurate appointment calendars for all clinicians.
  • Obtain, verify, and update patient demographic and insurance information in the scheduling software.
  • Contact patients prior to scheduled appointments to obtain required referrals and documentation.
  • Coordinate patient referrals as needed to support continuity of care.
  • Verify insurance authorizations prior to scheduled appointments to ensure eligibility and coverage.
  • Create and prepare patient charts for upcoming appointments.
  • Contact patients and families when additional information or documents are required prior to visits.
  • File patient charts and paperwork with Callier front desk staff in preparation for scheduled visits.
  • Serve as a liaison between clinicians and patients and their families to facilitate clear communication.
  • Create, update, and adjust clinician schedule templates as needed to support clinic operations.
    Balance daily patient collections and ensure accurate financial reconciliation.
  • Assist with financial screening for patients requesting gift funding or financial assistance.
  • Provide backup support to the Patient Account Specialist and Medical Schedulers as needed.
  • Perform other duties as assigned to support clinic and departmental operations.
