Schedule: Monday-Friday, 8:00 AM - 5:00 PM

Position Summary
We are seeking a compassionate and organized Licensed Practical Nurse (LPN) Case Manager to support patients with chronic health conditions through care coordination, patient education, and transitional care services. The ideal candidate will have case management experience and a strong commitment to improving patient outcomes.
Key Responsibilities
- Identify and support patients with chronic health conditions.
- Conduct patient assessments and medication reconciliation.
- Provide patient education and self-management support.
- Coordinate referrals and community resources.
- Assist patients transitioning from hospital to home.
- Communicate changes in patient condition to providers and supervising RN.
- Maintain accurate documentation in the electronic medical record (EMR).
- Participate in care coordination meetings and team huddles.
Qualifications
- Active, unrestricted LPN license.
- Previous Case Management experience required.
- Experience with chronic disease management and care coordination preferred.
- Strong communication and documentation skills.
- Proficiency with EMR systems.
- Ability to work collaboratively in a fast-paced healthcare environment.
Preferred Experience
- Transitional care or discharge planning.
- Motivational interviewing.
- Population health or chronic care management.
- Knowledge of community resources and support services.
