Registered Nurse (RN) - Home Health
Position Overview
We are seeking a compassionate and experienced Registered Nurse (RN) to join our growing Home Health team serving Dallas and surrounding areas. This role is responsible for providing skilled nursing care to patients in their homes, conducting comprehensive assessments, completing OASIS documentation, and managing admissions while ensuring high-quality patient outcomes.
Schedule & Coverage
- Schedule: Monday - Friday, 8:00 AM - 4:30 PM
- On-Call/Weekend Requirement: Yes
- Coverage Area: Dallas, TX and surrounding communities
- Weekly Hours: 40 hours
- Productivity Expectation: 30
- Mileage Reimbursement: Yes, travel between patient visits is reimbursed
Key Responsibilities
- Conduct skilled nursing visits in patients' homes.
- Perform comprehensive patient assessments and develop individualized care plans.
- Complete and maintain accurate OASIS documentation.
- Manage and perform patient admissions, including intake assessments and care coordination.
- Administer medications and treatments as prescribed.
- Monitor patient progress and identify changes in condition.
- Collaborate with physicians, therapists, caregivers, and interdisciplinary healthcare teams.
- Educate patients and families on disease management, medications, and home care plans.
- Ensure compliance with all agency, state, federal, and regulatory requirements.
- Maintain timely and accurate electronic medical records.
Required Qualifications
- Active and unrestricted Registered Nurse (RN) license.
- Home health nursing experience.
- Experience completing OASIS assessments and admissions.
- Strong clinical assessment, documentation, and patient care skills.
- Valid driver's license, reliable transportation, and ability to travel throughout the coverage area.
