Title: Lifestyle Health Coach

Job Responsibilities:

• Manage a large caseload of individuals living with Type 2 Diabetes through telephonic, chat, and virtual coaching interactions.
• Conduct structured coaching sessions focused on sustainable health and wellness behavior change.
• Utilize motivational interviewing, active listening, and evidence-based coaching techniques to support participant goals.
• Help individuals identify strengths, overcome barriers, and develop realistic action plans for improving health outcomes.
• Review and interpret health data from digital monitoring tools to provide personalized lifestyle guidance.
• Support participants in areas such as nutrition, physical activity, sleep, stress management, and medication adherence.
• Collaborate with a multidisciplinary care team including physicians, nurses, dietitians, and care coordinators.
• Document participant interactions accurately and maintain compliance with HIPAA and company policies.
• Communicate effectively through phone, email, chat, and video platforms.
• Participate in team meetings, training sessions, quality initiatives, and process improvement efforts.
• Meet performance goals related to member engagement, satisfaction, productivity, quality scores, and schedule adherence.
• Adapt to evolving technology platforms and healthcare program requirements.

Must Have/Required:

• National Board Certified Health & Wellness Coach (NBC-HWC) certification or actively working toward certification.
• Minimum 1 year of health coaching, wellness coaching, behavior change coaching, or related experience.
• Experience conducting coaching sessions virtually, telephonically, or through digital platforms.
• Strong motivational interviewing and behavior change skills.
• Excellent written and verbal communication skills.
• Proficiency with Microsoft Office applications (Word, Excel, Teams) and web-based systems.
• Experience working in a HIPAA-compliant healthcare environment.
• Ability to manage a high-volume member caseload while maintaining quality and customer satisfaction.
• Comfortable using digital health tools and learning new technologies.
• Strong organizational, time management, and documentation skills.

Preferred Qualifications:

• Bachelor's degree or higher in Health, Nutrition, Wellness, Exercise Science, Psychology, Public Health, or a related field.
• Additional wellness, fitness, nutrition, or coaching certifications.
• Experience supporting individuals with chronic conditions, especially Type 2 Diabetes.
• Experience working within a multidisciplinary healthcare team.
