Title: Registered Dietitian II (Virtual Diabetes Care Program)
Location: Remote
Job Type: Contract
Job Responsibilities:
- Conduct virtual nutrition consultations via phone and video platforms for members managing Type 2 diabetes and related chronic conditions.
- Assess members' nutritional needs based on medical history, lifestyle, biometrics, and health goals.
- Analyze Continuous Glucose Monitor (CGM) data and other health-related metrics to provide personalized recommendations.
- Deliver evidence-based Medical Nutrition Therapy (MNT) for diabetes, cardiovascular health, chronic kidney disease, weight management, carb counting, and healthy eating patterns.
- Utilize motivational interviewing, coaching techniques, and behavior-change strategies to support long-term lifestyle improvements.
- Lead individual and group education/coaching sessions focused on nutrition and health behavior change.
- Collaborate with multidisciplinary care teams including nurses, providers, health coaches, and support staff to optimize member outcomes.
- Monitor member progress and adjust nutrition care plans as needed.
- Document assessments, interventions, and outcomes accurately within electronic health record (EHR) systems.
- Contribute to educational content, training materials, newsletters, and other program initiatives.
- Maintain professional licensure and participate in ongoing learning and development activities.
- Support a fast-paced virtual healthcare environment while delivering high-quality member experiences.
Must Have/Required:
- Bachelor's degree or higher in Nutrition, Dietetics, or a related field.
- Active Registered Dietitian (RD) credential.
- Licensed Dietitian in at least one U.S. state.
- Minimum 1 year of experience as a Registered Dietitian.
- Minimum 1 year of coaching, counseling, or behavior-change experience with demonstrated success.
- Experience providing virtual care through video conferencing, telehealth, or telephone-based consultations.
- Knowledge of diabetes management and Medical Nutrition Therapy (MNT).
- Ability to interpret and educate patients using CGM data and other health metrics.
- Experience working in a HIPAA-compliant healthcare environment.
- Strong verbal and written communication skills.
- Proficiency with Microsoft Office applications and web-based healthcare systems.
- Willingness to obtain additional state licenses if required.
- Ability to work independently in a remote environment with a secure, dedicated workspace and reliable high-speed internet.
Preferred Qualifications:
- National Board-Certified Health & Wellness Coach (NBC-HWC) certification or pursuing certification.
- CDCES (Certified Diabetes Care and Education Specialist) or other relevant wellness/health certifications.
- Experience with virtual group coaching and health education programs.
- Experience working in chronic disease management and digital health/telehealth settings.
