Position Overview

The Care Extender - Enhanced Care Management (ECM) serves as a liaison for Enhanced Care Management referrals, supporting members by connecting them with healthcare and community resources. Under the direction of the Manager, Integrated Care Management, this role is responsible for member outreach, eligibility screening, referral coordination, and documentation across multiple care management platforms. The ideal candidate has strong customer service skills, excellent communication abilities, and a passion for assisting underserved populations.


Key Responsibilities

  • Manage incoming Enhanced Care Management (ECM) referrals.
  • Conduct member outreach via phone, text, email, and written correspondence.
  • Screen members for ECM eligibility and coordinate referrals to Community Based Care Management Entities (CBCMEs).
  • Collaborate with providers, community organizations, and internal departments.
  • Support operational activities including reporting, data tracking, and process improvement initiatives.
  • Assist care coordinators and care management teams with member support and case routing.
  • Participate in multidisciplinary care team activities to ensure coordinated member care.
  • Utilize Motivational Interviewing and other engagement techniques to effectively communicate with members.
  • Maintain accurate and timely documentation in care management systems.
  • Coordinate access to medical, behavioral health, specialty, and ancillary services.
  • Support quality improvement initiatives, including HEDIS gap closure efforts.
  • Build and maintain strong relationships with providers, community partners, and stakeholders.
  • Attend team meetings, trainings, and departmental projects.
  • Address escalated member concerns professionally and effectively.
  • Perform additional duties as assigned.

Required Qualifications

  • High School Diploma or GED required.
  • Minimum 2 years of customer service experience.
  • Experience working in a team-oriented environment.
  • Data entry and documentation experience.
  • Strong organizational and multitasking abilities.
  • Minimum typing speed of 45 WPM.
  • Proficiency with Microsoft Windows and standard business applications.
  • Excellent verbal and written communication skills.
  • Ability to work independently with minimal supervision.

Preferred Qualifications

  • Experience in healthcare, managed care, care management, health plans, social services, or community outreach.
  • Valid California Driver's License preferred.
  • Experience with Motivational Interviewing or similar member engagement techniques.
  • Experience supporting behavioral health, mental health, substance use, or vulnerable populations.
  • Familiarity with care management systems and healthcare documentation processes.
