Our client is currently seeking a Healthy Planet Analyst to work on a contract basis.
Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
Key Qualifications
• Epic Healthy Planet Certification required
• Experience supporting end of year reporting and quality measure reporting initiatives
• Experience with discharge planning and Care Management workflows within Epic Compass Rose
• Strong understanding of population health and care coordination processes
• Ability to work with operational and clinical stakeholders to support workflow optimization
Preferred Qualifications
• Epic Ambulatory Certification
• Experience supporting ambulatory clinic workflows and related Epic applications
• Prior healthcare system experience with population health and value-based care initiatives
