Pay Rate: $45-$47/hour
Start Date: September 14 (Firm Start Date)
Duration: 3-Month Contract with Potential Extensions

Equipment: Candidate must provide their own equipment or equipment will be provided by Judge Group
About the Role
We are seeking an experienced Health Coach (RN) to serve as the primary clinical resource for members across the full continuum of care, including wellness promotion, disease prevention, chronic condition management, and complex care support. In this role, you will leverage clinical expertise, care management principles, and motivational interviewing techniques to empower members to achieve their health goals and improve overall health outcomes.
As a trusted partner to members, families, providers, and healthcare teams, you will assess needs, develop personalized care plans, coordinate services, provide education, and advocate for quality, cost-effective care.
What You'll Do
- Serve as the primary nurse for members with a broad range of healthcare needs, from wellness and prevention initiatives to chronic and complex medical conditions.
- Provide health coaching, case management, and care coordination services to support positive health outcomes and self-management.
- Conduct comprehensive health assessments and identify opportunities for intervention.
- Develop individualized care plans with measurable goals and evidence-based interventions.
- Utilize motivational interviewing techniques to encourage behavior change and member engagement.
- Collaborate with members, families, caregivers, physicians, and healthcare providers to address barriers to care and treatment adherence.
- Maintain regular telephonic outreach and follow-up to monitor progress and adjust care plans as needed.
- Promote preventive care, wellness initiatives, and healthy lifestyle choices.
- Coordinate care across multidisciplinary teams to ensure seamless, patient-centered service delivery.
- Connect members with community resources, support programs, and health plan benefits.
- Monitor service quality, identify improvement opportunities, and escalate safety concerns when appropriate.
- Maintain accurate and timely documentation in electronic systems.
- Ensure compliance with all regulatory, accreditation, and organizational requirements.
Required Qualifications
- Active Pennsylvania RN License or Compact RN License valid in Pennsylvania
- Minimum 3 years of experience in one or more of the following:
- Clinical Nursing
- Case Management
- Disease/Condition Management
- Strong clinical assessment and critical thinking skills
- Excellent communication, relationship-building, and problem-solving abilities
- Proficiency with electronic documentation systems and basic computer applications
Preferred Qualifications
- Experience with health coaching and member-centered care
- Background utilizing motivational interviewing techniques
- Case Management certification (CCM) is a plus
