Job Summary

DFW Denton is seeking an experienced Travel Registered Nurse (RN) to provide skilled home health nursing care throughout Denton County. The ideal candidate will be proficient in Homecare Homebase (HCHB) and experienced in delivering comprehensive patient care in the home setting.

Key Responsibilities

• Provide skilled nursing care in patients' homes, including assessments, treatment planning, and ongoing clinical evaluations.
• Perform wound care, medication management, patient and caregiver education, and disease management.
• Complete OASIS assessments and maintain accurate, timely documentation in Homecare Homebase (HCHB).
• Monitor patient progress and communicate changes in condition to physicians and the interdisciplinary care team.
• Coordinate care to ensure optimal patient outcomes and support patient independence at home.
• Ensure compliance with Medicare regulations, agency policies, and quality standards.
• Participate in on-call and weekend coverage as required.

Required Qualifications

• Active Registered Nurse (RN) license.
• Current BLS Certification.
• Proficiency with Homecare Homebase (HCHB).
• Home Health nursing experience.
• Strong assessment, documentation, and care coordination skills.
• Valid driver's license and reliable transportation.

Preferred Qualifications

• Extensive Home Health experience.
• Strong OASIS documentation skills.
• Excellent communication, organization, and time-management abilities.
  

Dress Code: Navy Blue or Gray Scrubs.
