Key Responsibilities

• Provide skilled nursing care to adult and geriatric patients in the home health setting.
• Perform patient assessments and deliver care in accordance with LPN scope of practice.
• Maintain patient confidentiality and adhere to all nursing practice standards.
• Complete accurate and timely documentation using HCHB (Homecare Homebase).
• Communicate effectively with physicians, patients, caregivers, and the interdisciplinary team.
• Support continuity of care and patient education.
• Monitor patient conditions and report changes as appropriate.

Required Qualifications

• Active LPN license in the state of practice.
• Current CPR certification.
• Home Health experience required.
• HCHB (Homecare Homebase) experience required.
• Valid driver's license.
