Registered Nurse (RN) – Home Health
Location: Dallas, TX & Surrounding Areas
Schedule: Monday–Friday, 8:00 AM – 4:30 PM
Mileage Reimbursement: Yes (between patient visits)
Key Responsibilities
- Provide and direct skilled nursing care in patients' homes in accordance with physician orders and agency policies.
- Perform patient assessments and develop, implement, and update individualized care plans.
- Coordinate patient care with physicians, therapists, caregivers, and other healthcare professionals.
- Monitor patient progress, document care accurately, and communicate changes in condition.
- Educate patients and families on disease management, medications, and home care needs.
- Maintain compliance with all state, federal, and agency regulations in a Point of Care setting.
Requirement
- Active RN license in the state of practice
- Current CPR certification
- Homecare Homebase (HCHB) experience
- Valid driver's license, vehicle insurance, and reliable transportation
