Remote Urgent Care Nurse Practitioner (Telehealth) – Full Time
Location: Remote Ohio OH or Connecticut CT
Schedule: Full-Time | 40 hours/week | Rotating shifts (days, nights, weekends, holidays)

Overview
Seeking a Remote Urgent Care Nurse Practitioner to deliver telehealth-based acute care services as part of an integrated care team. This role supports 24/7 coverage and focuses on virtual patient management across multiple settings.
Key Responsibilities
- Provide telephonic and video-based urgent care to patients, caregivers, and facility staff
- Perform assessments, manage acute/chronic conditions, and document encounters in EMR
- Coordinate care across providers, facilities, and care teams
- Support care transitions and adjust plans based on patient needs
- Conduct advanced care planning discussions when appropriate
- Ensure documentation meets CMS and organizational compliance standards
- Participate in quality initiatives and ongoing training
Schedule & Coverage
- 24/7 program coverage
- Majority of weekday shifts: evenings/nights (5pm–8am)
- Every other weekend required (8–12 hour shifts, day/night rotation)
- ~26 weekends/year required (PTO limitations apply)
- Rotating holiday coverage
Required Qualifications
- Active NP license (unrestricted) in home state + ability to obtain additional licenses
- Graduate of accredited NP program (MSN or DNP)
- Board Certification: FNP, ANP, GNP, or AGACNP (AANP or ANCC)
- DEA license or ability to obtain
- Eligible for collaborative practice agreement (if required by state)
- Strong EMR and computer skills
- Ability to work assigned shifts and maintain schedule reliability
Preferred Qualifications
- 3+ years NP clinical experience
- Experience with geriatrics, chronic illness, or complex patient populations
- Background in telehealth, urgent care, SNF, or acute care
- Strong communication and patient-facing skills
- Comfort with end-of-life and advanced care discussions

- Fully remote role – must work in a private, secure environment
- Requires flexibility to support multiple regions and teams
