Our client is currently seeking a Clinical Post Acute Care (PAC) SME
Title: Clinical Post‑Acute Care (PAC) Subject Matter Expert / Manager
Location: Remote with 50%, primarily Midwest
Job Type: 5‑month contract engagement
Job Responsibilities:
- Serve as a contract Post‑Acute Care (PAC) Subject Matter Expert, supporting inpatient case management and clinical variation initiatives for large healthcare system clients.
- Provide hands‑on advisory leadership to design, implement, and optimize post‑acute care programs that improve care delivery, utilization management, and post‑discharge outcomes.
- Partner closely with client leadership, clinicians, and operational stakeholders to support strategic post‑acute transformation initiatives.
- Lead program and engagement management activities, ensuring defined project objectives, milestones, and outcomes are achieved within the engagement timeline.
- Collaborate with cross‑functional internal teams and client stakeholders across multiple workstreams to drive consistency, alignment, and high‑quality delivery.
- Analyze post‑acute performance data and workflows to identify opportunities to improve care efficiency, utilization patterns, and financial sustainability.
- Facilitate client meetings, executive readouts, and working sessions; develop clear, data‑driven recommendations and presentations for senior leadership.
- Support change management efforts to drive adoption of new clinical workflows, operational processes, and post‑acute care strategies.
- Act as a trusted advisor, maintaining strong relationships with clinical leaders, case management teams, and operational executives throughout the engagement.
Must Have / Required:
- 5+ years of experience in healthcare consulting, management consulting, hospital operations, care management, or project/program management.
- Strong post‑acute care domain knowledge, including SNFs, home health, IRFs, LTACHs, and transitions of care.
- Proven ability to manage multiple concurrent workstreams in fast‑paced, client‑facing environments with minimal supervision.
- Demonstrated strategic thinking and business acumen, with the ability to align clinical recommendations to organizational and financial goals.
- Advanced proficiency in MS Excel and PowerPoint for data analysis, reporting, and executive‑level presentations.
- Strong written and verbal communication skills, including experience presenting to senior and executive leadership.
- Proven track record of building effective relationships with clinicians, operational leaders, and business stakeholders.
- Experience working within large, matrixed organizations and collaborating across diverse internal and external teams.
- Adaptable, flexible, and comfortable navigating ambiguity and change.
- Willingness and ability to travel regionally up to 50–80% during the engagement.
Nice to Have / Preferred:
- Licensed Nursing Home Administrator (NHA) or similar clinical/administrative credential.
- Experience leading clinical transformation or post‑acute optimization initiatives that delivered measurable and recurring financial impact.
- Background in project turnaround, remediation, or change management methodologies.
- Demonstrated leadership experience mentoring or guiding cross‑functional teams.
