Job Title
Coding Specialist (CPT / HCPCS / ICD‑10)
Job Summary
We are seeking a Coding Specialist responsible for the timely review of provider-submitted charges and the accurate assignment of procedural and diagnostic codes. This role ensures proper billing, compliant claim submission, and effective reimbursement to the health plan by applying CPT, HCPCS, ICD‑10, and HCC coding standards.
Key Responsibilities
- Review provider documentation to abstract, assign, and enter accurate ICD‑10, CPT, and HCPCS codes into the electronic health record (EHR) system
- Perform medical chart reviews to identify, validate, and correct missed or inaccurate HCC codes in accordance with risk‑adjustment requirements
- Collaborate with providers to obtain clarification and provide feedback to resolve coding discrepancies
- Enter charges into the practice management system accurately and timely
- Submit electronic claims within 48–72 hours of charge entry
- Complete additional tasks and duties as assigned
Required Qualifications
Education
- High School Diploma or equivalent (required)
Experience
- 0–2 years of coding experience in a healthcare or clinical setting
- Experience with medical chart review and provider documentation preferred
Preferred Skills & Knowledge
- Working knowledge of ICD‑10‑CM, CPT, and HCPCS coding guidelines
- Familiarity with HCC coding concepts and medical chart abstraction
- Experience using EHR systems (Epic preferred) and practice management platforms
- Strong attention to detail with a focus on accuracy and timeliness
- Ability to communicate effectively with providers and internal teams
- Strong organizational and time‑management skills
- Ability to work independently while meeting deadlines and productivity expectations
