Job Title
Senior Living Care Navigator (SLCN)
Job Summary
The Senior Living Care Navigator is a non-clinical, non-licensed healthcare role responsible for telephonic outreach and support for senior living members. The SLCN introduces plan benefits, conducts assessments, identifies member needs, and collaborates with Optum APCs, RNs, facility staff, and responsible parties to support care coordination.
Work Schedule
Full-time (40 hours/week), Monday–Friday | 8:30 AM – 5:00 PM (member’s time zone)
Key Responsibilities
- Conduct telephonic outreach to new and existing members
- Introduce the care team and explain plan features and benefits
- Perform telephonic, SDOH, and RSA assessments
- Guide members on benefit utilization (transportation, OTC catalog)
- Connect members with the care team for coordination needs
- Maintain accurate documentation
Requirements
- High School Diploma/GED (required)
- 2+ years’ experience in a physician office, SNF, ALF, or outpatient setting
- CNA, CMA, or LPN background preferred
- Strong communication and organizational skills
- Proficiency in Microsoft Word, Excel, and Outlook
- Available Monday–Friday
