Contract Physical Therapist – Home Health/Hospice

Overview

We are seeking an experienced Travel Physical Therapist to support home health and hospice patients across the Dallas service area. This role is ideal for clinicians who thrive in autonomous field‑based work, enjoy managing a diverse caseload, and are comfortable working in high‑volume home health environments.

Schedule & Work Expectations

• Weekly Hours Guaranteed: 32

• Shift: Monday–Friday, 8:00 AM–4:30 PM

• On‑Call: 1 weekend per month

• Mileage Reimbursement: Yes, from first patient to last patient

• Case Conference: Thursdays at 9:00 AM

Key Responsibilities

• Complete 30 productivity points per week, including OASIS assessments and treatment visits

• Submit all documentation within 24 hours

• Provide clinical oversight and supervision to PTAs

• Deliver high‑quality, patient‑centered care in a home health/hospice setting

• Collaborate with interdisciplinary teams to support patient progress and care planning

Required Qualifications

• Master’s degree in Physical Therapy

• Active Physical Therapist license

• Home Health experience (minimum 2 years preferred)

• HCHB (HomeCare HomeBase) experience

• Ability to work independently in the field and manage a dynamic caseload

Preferred Skills

• Experience in high‑volume home health agencies

• Experience with high‑acuity patient populations
