Remote LPN – Call Center Support (Home Visits & Telehealth)

Schedule: Monday–Friday, 9:00 AM–6:00 PM EST
Location: Remote (U.S.)

We’re seeking a detail-oriented and patient-focused Remote LPN to join our care coordination team. This role is ideal for someone who is comfortable in a high-volume outbound call environment and thrives in supporting providers by preparing patient charts and conducting assessments prior to scheduled home visits and telehealth appointments.

Key Responsibilities

In this position, you will play a critical part in ensuring patients are prepared for their upcoming visits. Daily responsibilities include:

1. Outbound Patient Outreach

Make consistent outbound calls to patients who are scheduled for home visits or telehealth visits.

Confirm appointment details, verify availability, and assist with any pre-visit questions.

2. Chart Preparation (Chart Prep)

Initiate chart prep by gathering clinical, demographic, and visit-relevant information from the patient.

Document pre-visit information to support providers ahead of scheduled appointments.

3. EMR Data Entry

Accurately enter patient data, updates, and assessment findings into the Electronic Medical Record (EMR).

Ensure all documentation meets internal quality standards and compliance requirements.

4. Phone-Based Patient Assessments

Conduct structured patient assessments over the phone to collect clinical information.

Identify potential needs, gaps in care, or follow-up items to support the provider during the visit.

5. Collaboration & Communication

Communicate any patient concerns or escalations to the clinical team promptly.

Work closely with providers to ensure charts are complete and accurate before each appointment.

Required Skills & Qualifications

  • Active LPN license (if applicable based on the role specifics)
  • Experience in a call center, care coordination, or ambulatory clinical setting preferred
  • Strong communication skills and comfort making a high volume of outbound calls
  • Ability to multitask and maintain accuracy in a fast-paced environment
